Q: Overheating

I have a 96 dodge ram 1500 that is running extremely hot. In town stopping and going the temp is around 200, but on the interstate doing 75-80mph the temperature is 230 to 255 (260 is the redline). Ive replaced the waterpump, thermostat, fan clutch and flushed the system numerous times. The heat in the cab is not very hot just luke warm and also switches from the front vents to defrosters under hard acceleration. Its about 40 degrees F outside.
