Good Afternoon , That is a lot of miles . Once you know its an internal problem, you have to change modes and decide if its something that you want to live with till it breaks or are you going to rebuild or replace the engine .

Would this issue definitely require a full engine rebuild?

It may not , and you could probably start tearing into the engine to find the problem just depends how far you want to go with it . But it becomes more of an issue of what do you want to do with the truck , and what it will cost and how is your money best spent . I don't know where you are located , but i can get a used engine with 150k for $650 on car-part.com. Can you have a good used engine installed for $2500 vs putting $1000 into a high mileage engine .

Here is a forum with the same issue ,if you read all through it one person with the issue hadams has the same problem and after checking it comes to the conclusion that it is a sticking lifter making the noise.
