Without knowing all of the information like the vehicle history, the components that have been tested or replaced, and the results of any tests that need to be run, I can only give you a general answer to your question. If any of the engine timing components like the chain/belt is off this can cause internal engine damage which will cause this issue. I recommend having the engine timing components mechanically checked to see if any of the components are damaged or worn needing to be replaced and to see if anything is off on alignment. If an issue is found then the engine will need to be checked over further to see if there is any internal engine damage as the engine itself may have damage due to the timing components being off or failing to where the engine has to be repaired or replaced.
