Q: Bad muffler sound that gets better after a few miles

Engine used to sputter, that quit when exhaust starated to sound as if was fallen off. After driving 3 or 4 miles the sound almost totally disappears. I feel a vibration quite strong, maybe a hangar of sorts?

My car has 220000 miles.
