Q: Rear hub has some play

Ive had some clunking noise when applying the brake so I took the wheels off to see if I could find something. What I found was that both rear wheel hubs have play in them (they move toward and away from axle). 2 questions Can that be the wheel bearing or the entire hub assembly that needs replaced? Would that have been the cause of the clunking noise?
