Q: P0421 code

Hello, I have an 02 Eclipse 2.4L with a Manual transmission. The SES light has been on for some time, I bought an OBD2 scanner to see what was the problem, it is throwing 2 P0421 codes. Both are the same code, 1 is pending and the other is confirmed. When accelerating I notice it wants to hesitate and all my power isn't there, and every once in a while it will stall out entirely.

My car has 230000 miles.
