Q: Own a 1989 F150, unusual rattling sounds in R and D, not in P or N, sound is back when giving it gas but goes away when cruising

My truck sounds normal when you start it up or when it is in Neutral, but as soon as you put it in Reverse or Drive, a very strange rattling/ clicking sound occurs almost like lifters that have no oil (oil level and pressure are fine), sound goes away when cruising but comes back when I am stopped. I have a suspicion that it might be an exhaust leak but I am no mechanic. Any help is greatly appreciated!
