Q: Overheating

Car overheats when I drive a short distance. I'm constantly having to fill it up with water. It looks like the resevoir may have a leak. When it starts to overheat the car will start smoking through my vents and through my dashboard where the speedometer and gauges are at.

My car has 142000 miles.
