Q: My belt keeps coming off. But it starts up fine. What's the problem???

My car was fine while driving one day and soon as I went to pull off my steering wheel locked up and I couldn't steer it at all. Now it's where I had a family friend to put it back on but a new belt and it keeps jumping off. Why???

My car has 150000 miles.
