Q: Ford 5.4 knocking noise under load or high rev 2007 Ford E-250

My 2007 Ford E-250 Ext Cargo van just recenter started to knock pulling a load up a hill or reving before shifting gears, doesn't tick or run any different untill a load, I've always pulled my 12' Ft. Enclosed trailer behind it, it's got 188, 000 miles now, I bought it 3 years ago with original 66, 000 miles, I travel the western mid west highways year round. Every one is saying stuck valve, possible something to do with the cam, it's the 5.4 3 valve, we have determined it's coming from the right front bank valve cover. Anything please Help, just would like to know what's happened to it. Thanks Darren
