Q: Engine revs when I turn it on and doesn't go back down.

When I turn on my car the engine revs up and won't go down. When I put it in reverse it mellows out slightly but doesn't completely stop. When driving I have to hold the gas or it'll keep accelerating. Got up to 40 mph at one point before pressing the break. What could this be? And how much would it cost to fix?

My car has 230000 miles.
